Just research, or just plain exploitation : Translational clinical
trials and the developing world

Abstract:

In numerous instances, researchers in high-income countries have
recruited subjects from low and middle-income countries into small
scale, first-in-human trials of highly innovative treatment strategies
like those involving gene transfer. However, discussions about the
ethics of this practice have been overshadowed by debate over the
ethics of developing world trials involving large-scale, late phase
clinical research. Various international guidelines articulate two
criteria for conducting clinical research in resource-poor settings:
post-trial access to study interventions and responsiveness to host
community health needs. After describing the difficulty of applying
the former to first-in-human studies, I defend the more fundamental
requirement of responsiveness to host community members. Ethical
criteria are advanced for fulfilling the condition of responsiveness;
additionally, recommendations are offered for translational
researchers and policy makers who might recruit trial volunteers in
low and middle-income countries.
